Individualized Services & Support Team (ISST) Facilitator

Work Schedule: Part-time/hourly (10-20 hours per week)

Exemption Status: Non-Exempt

Annual Range: $22-$27/hr DOE

Service Area: San Miguel and West Montrose Counties

Position Summary:

Bright Futures is seeking a facilitator and case manager for our Individualized Services and Support Team (ISST; see description below). This position is responsible for coordinating and maintaining all records associated with ISST meetings. This position fields all referrals for the Individual Services and Support Team facilitates the ISST meetings, works directly with families through the ISST process and provides case management as needed.

Essential Duties and Responsibilities:

· Receive referrals, coordinate and facilitate ISST meetings which includes documenting group discussion indicating areas of focus and maintaining confidential meetings notes and records

· Contact families, partner agencies and other supports to arrange meeting logistics

· Provide direct case management as needed

· Coordinate/provide oversight to program and ensure quality and fidelity

· Create and distribute monthly family meeting notes to each stakeholder in a timely manner

· Act as liaison between the agency partners and maintain relationships to ensure communication and integration of services

· Track Collaborative Management Program goals and report to required entities

· Promote and educate the community about integrated services

· Enter data into database

· Maintain confidentiality in all settings

· Participate in statewide calls and meetings

· Complete Annual reports and Memorandum of Understanding with community partners

· Complete training in relevant services

· Other duties as assigned

Qualifications and Requirements:

· Strongest candidates will have minimum of a BA in a field related to human services (social worker, counselor, teacher, administrator, etc.)

· Five years’ experience within the human services field

· Case management skills

· Time management skills

· Knowledge of resources, government programs and family systems

· Outstanding verbal and written communication and organizational skills

· Strong communication skills, both verbal and written

· Ability to multi-task, problem solve and work in a team

· Willingness to accept feedback and adapt accordingly

· Flexibility and ability to adapt to changing circumstances

· Commitment to ongoing learning of diversity, equity, and inclusion

· Proficiency with email, and other computer related programs (google docs, google calendar, Excel, Zoom and other databases as needed)

· Ability to work independently

· Background check, including fingerprints must be completed

· Must have own transportation and current Colorado Driver’s License and Insurances

Program Description:

Collaborative Management Program (CMP) The purpose of the Collaborative Management Program (CMP) is to effectively and efficiently collaborate and share resources, and to manage and integrate the support and services provided to children and families who benefit from multi-agency services.

Key goals of CMP:

Reducing the duplication and eliminate fragmentation of services

Increasing the quality, appropriateness and effectiveness of services provided

Encouraging cost-sharing among service providers

Encouraging active participation of parent advocacy or family advocacy organizations.

Our Collaborative Management Oversite Group has chosen to use an Interagency Support Services Team, along with other programs housed within other organizations, to meet the goals of the CMP.

Interagency Services and Support Team (ISST)

Individual Service and Support Teams (ISST) are interagency teams that support families and youth ages 0-18 in San Miguel, Ouray and West Montrose Counties.

Each ISST is uniquely formed to best serve individual families. They are made up of community agencies, schools, health and human services and other organizations that the family is engaged with or could utilize services from.

ISST meets monthly with the support of a facilitator to help families identify their strengths, needs, and develop a collaborative service plan. Service planning can include brainstorming options, troubleshooting challenges, connecting families to community resources or mental and behavioral health providers.
